Why are white lesions white?

A

Something blocking redness of underlying vasculature

  1. Thickening of epitherlium: hyperkeratosis, acanthosis, dysplasia, carcinoma
  2. Decreased vascularity: anemia
  3. Increased collagen content of the sub epithelial tissue: sub mucous Fibrosis

microbial colony, necrosis

Is leukoplakia considered a “premalignant lesion”

A

Yes, 5-25% are dysplastic or worse at time of biopsy.
4% potential to transform if you do nothing.
90% of floor of mouth are malignant

How should you approach a suspected leukoplakia?

A

Attempt to scrap it off, if it does not scrap off, observe for 10-14 d. and than biopsy it

What is Linea Alba? how do we Dx and Tx it?

A

Clinical Dx: white, cannot be wiped off, horizontal, elevated, linear lesion, near midline of buccal mucosa, commonly seen bilaterally

Cause: stress, sucking

Tx: none

A

Clinical Dx: White, can’t be wiped off, common in African American population, commonly seen bilaterally, disappears when stretch

Can also appear blue or grey, particularly found on the buccal mucosa; it may also occur on the mucosa of the larynx or vagina.
