Why are white lesions white?
Something blocking redness of underlying vasculature
- Thickening of epitherlium: hyperkeratosis, acanthosis, dysplasia, carcinoma
- Decreased vascularity: anemia
- Increased collagen content of the sub epithelial tissue: sub mucous Fibrosis
Others:
microbial colony, necrosis
Is leukoplakia considered a “premalignant lesion”
Yes, 5-25% are dysplastic or worse at time of biopsy.
4% potential to transform if you do nothing.
90% of floor of mouth are malignant
How should you approach a suspected leukoplakia?
Attempt to scrap it off, if it does not scrap off, observe for 10-14 d. and than biopsy it
