Q

Collaboration involves a team (RN, CNA, all Physicians) that…

A
  1. Streamlines decision making process
  2. Allows achievement of goals
  3. Ensures continuity (only way we can ensure this is through the nurse. you ensure that everyone who goes in your patients room knows what everyone else is doing)

Q

Trends influencing CM (CIRRF)

A
  1. Cost of HC
  2. Information explosion (so many more services being offered and choices for consumers to consider)
  3. Responsibility shift to consumer and to HCP to help consumers understand (this is the expectation and how we can keep costs down by educating consumers)
  4. Reimbursement for providers
  5. Fragmentation of HC delivery (many services being offered beyond the hospital walls)

Q

CMs do not provide direct patient care, however because they are RNs, they are responsible for…

A

planning care (not advocating, coordinating or ensuring because planning is the only one out of these choices that is part of the nursing process. case managers are nurses!)

Q

What is the goal of case managment

A

achieving a balance between quality (quality of care has to be great, every time you see your patient you need to be making a difference. you do not want anyone readmitted in less than a month) and cost (when quality is improved cost is decreased)

Q

How does CM impact our health care system?

A

Helps keep costs down, makes more jobs for APRNs, focus is on finding out if this patient will have compliance issues (will they end up back in the hospital and cost more money due to lack of compliance with treatment plan),focus towards preventing duplication of services

Q

Barriers to continuity of care?

A
  1. attitude of HC worker (are you a tired out nurse? are you getting enough sleep? are you taking care of yourself? do not let yourself become overworked)
  2. client motivation
  3. lack of knowledge (about case managment, about continuity of care, professionals who let continuing education go to the wayside and haven’t learned about a new technique or something)
  4. Communication barriers (written and spoken and time)
  5. Trans cultural barriers
  6. reimbursement issues
  7. system failures (poor staffing, poor communication within system)

Q

documentation and communication

A
  1. consultations (when you need a second opinion or help from specialists they come and they want to know why they are here and wants needs to be done so you need correct documentation for that and correct data so they know wha they have to do and so the patient is safe and getting the right care)
  2. referrals (based on patient needs)
  3. transfers (from one unit to the next. you gotta be sure that when your patient is leaving your unit to go down to radiology that everything is okay. for example, check your patients blood sugar before they go to make sure they will be okay while they are getting xrays done)
  4. discharge planning (starts on admission in the ED)

Five Major Management Functions of the NURSE MANAGER (KNOW THESE. Page 293)

A
  1. Planning (1st step of nursing process. where are you going to put most of your resources? effective planning has to coincide with the position and philosophy of the organization. What goals and objectives are you going to establish? long term vision?)
  2. Organizing (using resources wisely, making correct assignments, coordinating activities with other departments, effective communication
  3. Staffing ( staffing process is the most time consuming and challenging functions for the nurse manager. this is what makes a unit cohesive or not. key to employee satisfaction, recruitment, and retention
  4. Directing (how are you going to get the job done? management by walking around in which the nurse manager literally walks around the unit with the primary purpose of interacting positively with the staff)
  5. Controlling (ensuring that employees accomplish goals
16
Q

Positive reinforcement (part of direction)

A

in the form of a sincere thank you, you did a good job. this is one of the most powerful yet most often underused motivational resources available to the manager. for it to be effective it needs to be specefic, occurring as close as possible to the time of the achievement, be spontaneous and unpredictable and not routine, be given for genuine accomplishment

17
Q

role modeling (part of direction)

A

as a manger you should be walking around the unit and doing things like answering call bells and showing your staff how you are supposed to act

18
