- Responsibility shift to consumer and to HCP to help consumers understand (this is the expectation and how we can keep costs down by educating consumers)

- Fragmentation of HC delivery (many services being offered beyond the hospital walls)

CMs do not provide direct patient care, however because they are RNs, they are responsible for…
planning care (not advocating, coordinating or ensuring because planning is the only one out of these choices that is part of the nursing process. case managers are nurses!)
What is the goal of case managment
achieving a balance between quality (quality of care has to be great, every time you see your patient you need to be making a difference. you do not want anyone readmitted in less than a month) and cost (when quality is improved cost is decreased)
Is continuity of care and collaborative care the same?
no, continuity of care is care from preadmission, to discharge to follow up and collaboration of care is more current/whats happening now.
How does CM impact our health care system?
Helps keep costs down, makes more jobs for APRNs, focus is on finding out if this patient will have compliance issues (will they end up back in the hospital and cost more money due to lack of compliance with treatment plan),focus towards preventing duplication of services
Barriers to continuity of care?
- attitude of HC worker (are you a tired out nurse? are you getting enough sleep? are you taking care of yourself? do not let yourself become overworked)
- client motivation
- lack of knowledge (about case managment, about continuity of care, professionals who let continuing education go to the wayside and haven’t learned about a new technique or something)
- Communication barriers (written and spoken and time)
- Trans cultural barriers
- reimbursement issues
- system failures (poor staffing, poor communication within system)
Nurses role in case managment (CAFELIO)
- coordinate

documentation and communication
- consultations (when you need a second opinion or help from specialists they come and they want to know why they are here and wants needs to be done so you need correct documentation for that and correct data so they know wha they have to do and so the patient is safe and getting the right care)
- referrals (based on patient needs)
- transfers (from one unit to the next. you gotta be sure that when your patient is leaving your unit to go down to radiology that everything is okay. for example, check your patients blood sugar before they go to make sure they will be okay while they are getting xrays done)
