Cognitive Level I
Automatic Actions
Conscious but profoundly disabled. Stares, may sit, walk or chew with simple commands.
OT: one action at at time - use physical contact to start an action
Cognitive Level II
Postural Actions
Actions related to comfort and discomfort.
Shortens attention span of less than 10 mins
Unaware of movements. Sits and initiates some gross motor actions.
Assume bizarre positions or perform strange-looking movements.
OT: use simple sensory motor activities - use familiar objects - repetition
Cognitive Level III
Manual Actions
Pointless manipulation of objects. Behaviors may be inappropriate or not goal-directed.
Capable of repetitive actions (spontaneous motor actions)
OT: enjoys repetitive hand movements - action more than product, have demonstration
Cognitive Level IV
GOAL DIRECTED
Goal directed with cues. Needs visual cues. what is not in plain sight is often ignored
OT: verbal directions, can use simple tools, motivation is by the product rather than just the motions

Exploratory Actions
Failure to think before acting. Trial and error problem solving. Impulsive or careless
OT: clients benefit from social skill training, learns most demonstrated skills, can use hand tools, can process trial and error mistakes

PLANNING AHEAD
Thinks before acting, can predict consequences of behavior. Has attention sensory cues, independent and organized

Precautions for Allen’s Cognitive Disability Model
- Do not label the person with a cognitive level since this level fluctuates
- Let the client do as much for himself as possible – just structure the environment to facilitate improved performance.
