A
T cell response to gluten in small bowel causes villous atrophy and malabsorption

A
- Steatorrhoea
- Diarrhoea
- Abdo pain
- Bloating
- N+V
- Aphthous ulcers
- Angular stomatitis
- WL
- Fatigue + weakness
- Osteomalacia
- FTT

what investigations would u do?
A
- Immunology:
- Tissue transglutaminase IgA abs
- Anti-endomysial abs - Duodenal biopsy:
- Villous atrophy
- increased intraepithelial lymph
- crypt hyperplasia
- Lamina propria infiltration w lymphocytes - Bloods:
- anaemia, low B12, ferritin

What is the management?
A
lifelong gluten free diet
avoid: wheat, bread pastry, pasta, barley, beer, rye, oats

What are the complications?
A
anaemia dermatitis herpetiformis osteopenia/porosis hyposplenism - offer pneumococcal vaccine GI T cell lymphoma increased risk of malignancy neuropathies
