What is coeliac disease?
A
- Autoimmune condition with sensitivity to gluten that causes inflammation in the small bowel.
- Inflammation is particularly in the jejunum and results in atrophy of the intestinal villi.

What are the symptoms of coeliac disease?
A
- Chronic or intermittent diarrhoea
- Failure to thrive (children)
- Persistent or unexplained GI symptoms including nausea and vomiting
- Prolonged fatigue
- Recurrent abdominal pain, cramping or distention
- Sudden or unexpected weight loss
- Unexplained iron-deficiency anaemia or other unspecified anaemia

What conditions are associated with coeliac disease?
A
- Autoimmune thyroid disease
- Dermatitis herpetiformis
- IBS
- T1DM
- 1st degree relatives with coeliac

What are the complications of coeliac disease?
A
- Anaemia (Iron, folate and B12 deficiency)
- Hyposplenism
- Osteoporosis, osteomalacia
- Lactose intolerance
- Enteropathy-associated T-cell lymphoma of the small intestine
- Subfertility, unfavourable pregnancy outcomes

How do you diagnose coeliac disease?
A
- Raised anti-TTG and anti-EMAS
- Endoscopy and biopsy:
Crypt hypertrophy
Villous atrophy

What may cause a false-negative result for coeliac disease?
A
IgA deficiency

When must investigations take place?
A
On a gluten diet

What is the management of coeliac disease?
A
- Lifelong gluten free diet
