Heparins (examples, routes, and action)
Examples: Heparin, Enoxaparin (Lovenox), Fondaparinux (Arixtra)
Route: IV (12 units/kg/hr) or SubQ (5000 units SQ Q12H)
Action: binds with antithrombin lll to inactive clotting factors
- inhibits production of thrombin
- prevents thrombus formation
- prevents production of fibrin
What is heparin used for?
Prevention of prophylactic
Post-stroke
PE/DVT
Acute MI
DIC
Blood clot risk during pregnancy
Heparin (SE and Education)
SE: Heparin-induced thrombocytopenia (HIT), Hemorrhage
Education: SubQ admin, adverse reactions, caution with injuries
Lovenox
Type of heparin (low molecular weight)
DVT prophylaxis, Prevents ischemia
High Cost
Given on a fixed dose schedule
No need for aPTT
Given SubQ (dose based on body weight)
Warfarin (uses, SE, and dosage)
Uses: DVT, PE, Afib, Prosthetic heart valves, and decreased risk of CVA/MI
SE: hemorrhage, N/V, abdominal pain, alopecia, dizziness, joint/muscle pain, urticaria
Dosage: varies on the INR level (usual starting dose: 5 to 10 mg and maintenance dose: 2 to 10 mg)
