Heparins (examples, routes, and action)

A

Examples: Heparin, Enoxaparin (Lovenox), Fondaparinux (Arixtra)
Route: IV (12 units/kg/hr) or SubQ (5000 units SQ Q12H)
Action: binds with antithrombin lll to inactive clotting factors
- inhibits production of thrombin
- prevents thrombus formation
- prevents production of fibrin

Warfarin (uses, SE, and dosage)

A

Uses: DVT, PE, Afib, Prosthetic heart valves, and decreased risk of CVA/MI
SE: hemorrhage, N/V, abdominal pain, alopecia, dizziness, joint/muscle pain, urticaria
Dosage: varies on the INR level (usual starting dose: 5 to 10 mg and maintenance dose: 2 to 10 mg)
