Transfer
A
- Wash hands. Explain procedure to client
- Refer to correct use of body mechanics
- Provide privacy
- Place wheelchair so the strong side of client will lead
- Place client’s slippers or shoes on their feet
- Lock bed brakes; lower bed to lowest level and raise head of
bed as far as client can tolerate - Turn client on side with shoulder/knee support
- Swing client’s legs over side of bed and at the same time
- Apply transfer belt snugly to client’s waist while client is sitting
- Stand in front of client to assess for balance and dizziness
- Ensure brakes locked on wheelchair & is within client’s reach
- Spread your feet apart and flex your hips and knees
- Assist client to slide buttocks to edge of bed until feet touch floor
- Brace your front knee against client’s weak leg as client stands
- Leading with good leg, face client, stand client up on 3 count
- Instruct client to place hands on chair armrest for support; flex
your knees and hips as you assist client into chair - Assess client’s alignment in chair and secure client as needed

Bedpan/PeriCare
A
- Wash hands. Explain procedure to client
- Prepare equipment
- Provide Privacy
- Don gloves
- Bring head of bed down
- Leader instructs assistant and client where to turn
- Turn client and place bed pan underneath
- Return client to back & bring head of the bed up for client comfort
- When client is finished bring head down
- Leader instructs assistant & client to turn keeping hand on pan
- Remove bedpan and set aside
- Wash front to back, clean to soiled (no double dipping)
- Handle Soiled materials appropriately, change under pad
- Remove soiled gloves
- Repositioned client
- Wash hands & document
