Transfer

A
  1. Wash hands. Explain procedure to client
  2. Refer to correct use of body mechanics
  3. Provide privacy
  4. Place wheelchair so the strong side of client will lead
  5. Place client’s slippers or shoes on their feet
  6. Lock bed brakes; lower bed to lowest level and raise head of
    bed as far as client can tolerate
  7. Turn client on side with shoulder/knee support
  8. Swing client’s legs over side of bed and at the same time
  9. Apply transfer belt snugly to client’s waist while client is sitting
  10. Stand in front of client to assess for balance and dizziness
  11. Ensure brakes locked on wheelchair & is within client’s reach
  12. Spread your feet apart and flex your hips and knees
  13. Assist client to slide buttocks to edge of bed until feet touch floor
  14. Brace your front knee against client’s weak leg as client stands
  15. Leading with good leg, face client, stand client up on 3 count
  16. Instruct client to place hands on chair armrest for support; flex
    your knees and hips as you assist client into chair
  17. Assess client’s alignment in chair and secure client as needed
  18. Wash hands & document

Bedpan/PeriCare

A
  1. Wash hands. Explain procedure to client
  2. Prepare equipment
  3. Provide Privacy
  4. Don gloves
  5. Bring head of bed down
  6. Leader instructs assistant and client where to turn
  7. Turn client and place bed pan underneath
  8. Return client to back & bring head of the bed up for client comfort
  9. When client is finished bring head down
  10. Leader instructs assistant & client to turn keeping hand on pan
  11. Remove bedpan and set aside
  12. Wash front to back, clean to soiled (no double dipping)
  13. Handle Soiled materials appropriately, change under pad
  14. Remove soiled gloves
  15. Repositioned client
  16. Wash hands & document
