CI- Olfactory
plug one nostril and have to identify the smell. “can you smell this? What are you smelling?”
CN II - Optic
Visual Acuity: Cover one eye, stand 20 ft away and read the smallest line on the Snellen Chart
Visual Fields: Cover one eye and PT moves object from left, right, down, and up stopping when pt indicates they can see the object. 2nd test: hold numbers up and have pt read the numbers
Pupillary reflex
Ipsilateral CN 3 Pupillary reflex not working
direct response of pupil dilation
Contralateral CN 3 Pupillary reflex not working
consensual response of pupil dilation
Ipsilateral CN 2 pupillary reflex not working
both direct and contract lateral dilation
CN III - Oculomotor
1) Extraocular movements: Moving eyes up, down, left and right
2) smooth pursuit: eyes follow the object left right up and down
3) convergence and divergence
4) Saccades: looking back and forth between objects
6) pupillary eye response
CN V - Trigeminal
1) light touch of above eyes, check, and jaw
2) pin prick of above eyes, check, and jaw
3) bite and palpate masseter
4) open jaw don’t close
5) jaw reflex
(also but won’t do: Touch of the cornea)
CN VII- Facial
1) forehead wrinkle and don’t let me move
2) close eyes tight and don’t let me open
3) show teeth and smile
4) fill mouth with air and taps
5)kissy face
6) pa pa pa
7) anterior 2/3 of tongue taste (won’t do)
CN VIII - Vestibulocochlear
Vestibular: turn head and testing acoustic nerve. Do you feel dizzy?
Cochlear division:
1) rubbing fingers can you hear this?
2) Webbers Test - Tuning fork on top of head
3) Rinne test- look at vibration on mastoid and air conduction of sound (which is louder)
CN IX - Glossopharyngeal (will not have to do on practical)
1) Kah kah kah
2) gag reflex
3) taste to posterior 1/3 of tongue
CN X - Vagus (will not have to do on practical)
“can you say ahh” should see uvula deviate side to side
CN XI - Accessory
1) shrug shoulder add resistance (traps)
2) turn head side to side add resistance (SCM)
