What is the correct sequence of the components of the clinician centered interview?
HPI, PMH, SH, FH, ROS
Describe the HPI
History of present illness is the most helpful component of health history. Describes both the symptoms of the possible disease and the personal/emotional context in which they occur
Describe the PMH
Used to document patient’s past and current health. Includes adult illnesses, childhood illnesses, medications, allergies and health maintenance
Describe the SH
Goal is to identify factors outside of past or current medical conditions that may influence the patient’s overall health
Describe the FH
Inquire about 1st degree relatives. Inquire whether anyone in the family has similar physical problems to the patient’s or if anyone at home has been ill lately with similar concerns
Describe the ROS
Lists and organizes most known symptoms per body system
Describe, define and identify clinician centered interview skills
- Closed ended data gathering skills (questions with yes, no or brief answers)
- Integrating patient centered & clinician centered skills (coning down: starting with an open ended question followed with a closed ended question)
- Symptom defining skills
Define and identify the differences between a sign and a symptom
A symptom is subjective and noticed by the patient (I.e. nausea, headache, pain).
A sign is objective and identified by the clinician (I.e. murmur, perforated TM, ascites).
