Primary- raised TSH, low FT3 and FT4
Subclinical- raised TSH, normal FT3 and FT4

Congenital Autoimmune thyroid disease (ie Hashimotos) Iatrogenic Chronic iodine deficiency Post-subacute thyroiditis
