A

The thyroid is the ONLY organ that can absorb iodine and it is necessary for both T3 and T4 throid hormones

What is the Negative feedback loop oof thyroid hormones?

A

Hypothalamus releases TSH which stimulates the pituatary gland to release TSH. This signals the thyroid to produce MORE T3 and T4. When there is A LOT of T3 and T4 this shuts off the release from the hypothalamus.

PMH: diabetes, hyperthyroidism, HTN

Medications: recent radioactive iodine treatment, Jardiance, Zestril

What is happening? What is the treatment?

A

Thyroid storm likely from the radioactive iodine treatment

Tx:
-antithyroid drug = PTU
-inorganic iodide (SSKI or Lugol’s)
- Beta blocker (propanolol)
- Dexamethasome
- Aggressive cooling

A patient comes to you to see if her medications are safe for pregnancy because she JUST found out she was pregnant. She is currently on Levoxyl 137 mcg once daily, Zoloft 50 mg once daily, and Metformin 1000 mg BID. What changes would you make, if any?

A

Levoxyl (levothyroxine) needs to be doses higher in pregnancy (30 - 50% higher)

137 x 1.5 = 205

200 mcg should be her new dose
***aggressive control of thyroid is important!!!

Metformin is safe
Zoloft is overall safe
