What is it
๐น Pain/cramping in muscles due to poor blood supply (ischemia), occurs with activity, relieved by rest.
๐น Most commonly due to peripheral artery disease (PAD).
Clinical presentation
๐น Intermittent muscle pain/cramping in legs (calves, thighs, buttocks).
๐น Triggered by exercise, relieved by rest.
๐น Other PAD signs: cool skin, weak pulses, hair loss, shiny skin
๐ข What does pain location indicate?
๐น Calf pain โ Superficial femoral artery disease.
๐น Thigh pain โ Common femoral artery disease.
๐น Buttock pain โ Aortoiliac disease (Leriche syndrome if with erectile dysfunction).
Epidemiology
๐น Affects ~5% of adults over 55.
๐น More common in men.
๐น Often underdiagnosed due to atypical symptoms.
Age groups affected
๐น Most common in adults over 50.
๐น Rare in young people unless they have risk factors.
Risk factors
๐น Atherosclerosis (main cause)
๐น Smoking ๐ฌ (strongest modifiable risk factor)
๐น Diabetes
๐น Hypertension
๐น High cholesterol
๐น Obesity
๐น Chronic kidney disease
๐น Sedentary lifestyle
Clinical features
๐น Diminished pulses ๐ซ๐
๐น Delayed capillary refill โณ
๐น Skin changes (shiny, hair loss, pallor, dependent rubor)
๐น Advanced cases: Ulcers, gangrene โ ๏ธ
Prognosis
๐น Progressive if untreated.
๐น 20-30% worsen in 5 years.
๐น 3-6x higher risk of heart attack/stroke.
๐น Severe cases โ limb amputation risk ๐ฆตโ ๏ธ
๐น Can improve with lifestyle changes & treatment
