class 2 antiarrythmic: BB Flashcards
effects of class 2 antiarrythmics
action via increasing cAMP=> decreases Na + ca current + suppress abnormal pacemaker/ decrease slope of PHASE 4 - > next ap delayed-> bradycardia (so an increase in RR interval)
drugs
metoprollol, propanolol, esmolol, sotalol
metoprolol
action: cardioselective b1, na ch inhibition, decrease risk
i: tachyarrythmias b/c increase in SYM, Afib, Aflutter, AVNRT, prevent ventricular arrythmia after MI
contraindication: heartblock, bradyarrythmia
AE: AV block, cold extremeties, impotence, hypercholesteremia, bradycardia, nightmares
propanolol
action: non selective, Na ch block, strongest
i: tachyarrythmias b/c increase in SYM, Afib, Aflutter, AVNRT, prevent ventricular arrythmia after MI
contraindication: heartblock, bradyarrythmia
AE: AV block, cold extremeties, impotence, hypercholesteremia, bradycardia, nightmares
esmolol
non - selective, short duration of action + fast onset, admin parenterally
i: tachyarrythmias b/c increase in SYM, Afib, Aflutter, AVNRT, prevent ventricular arrythmia after MI
contraindication: heartblock, bradyarrythmia
AE: AV block, cold extremeties, impotence, hypercholesteremia, bradycardia, nightmares
sotalol
non selective, k ch inhibitor
i: tachyarrythmias b/c increase in SYM, Afib, Aflutter, AVNRT, prevent ventricular arrythmia after MI
contraindication: heartblock, bradyarrythmia
