CIS: Platelet Disorders Flashcards
What is thrombocytosis?
> 450 platelets
- if there’s underlying disease, then it’s reactive
- if not, look for JAK2 genotyping
- t(9;22) is CML
What is the most common cause of bleeding?
thrombocytopenia
risk of bleeding is inversely proportional to the ….
platelet count
are patients with thrombocytosis symptomatic?
no
What is common in thrombocytopenia?
petechiae
What is more common in coagulopathy ?
a bruise… ecchymosis
hemarthrosis
bleeding into a joint
- common in hemophilia
- also seen in trauma, post operative states
Nosebleeds, where is the most common place for those to come from
Keiselbach’s plexus
Erythromelalgia, how is that different from raynauds pheomenon
- seen in ET
- it hurts, raynauds doesn’t
- good response to baby aspirin
what is pseudothrombocytopenia
in vitro clumping…. not in the patient
-repeat CBC in heparin or citrate
what are the 4 most common drugs in hospitalized patients that lower the platelet count?
- loop diuretics
- H2 blockers
- digoxin (this is tough because they usually have a heart problem if they are taking this)
- antibiotics
What happens with ITP again?
- more common in fall and winter months
- usually associated with upper respiratory infections
- easy bruisability and a rash on legs and had a cold about a month ago
Is bleeding inversely related to platelet count?
yes
-remember that
Do you have to Treat ITP if the platelet count is over 35 thousand and he only has petechiae?
no, just observe it
-petechiae will never hurt ya
If there is N/G tube draining coffee-ground material, what does that mean?
GI bleed so watch out
