Estadiamento Child-Pugh:
Bilirrubina

Indicações tratamento para hepatite B:
Aguda grave
Crônica: HBV-DNA≥ 2.000UI/ml + ALT* ≥ 37 (mulher) ou 52 (homem)
*ALT normal: reavaliar (Elastografia >9kPa? |Biópsia ≥ A2/F2?)
Tratamento hepatite B crônica (Tem, a lá! ou alfa te pega):
META: QUEDA DE HBV-DNA APÓS 6M
1ª opção: Tenofovir - TDF (contraindicado: doença renal ou óssea)
2ª: Entecavir - ETV (preferir se: cirrose, imunossupressão ou QT)
3ª TDF Alafenamina (TAF) - menos tóxico
ALTERNATIVA: alfapeg-interferon
Meta tto para HCV?
HCV-RNA negativo 12sem após tratamento
Anéis de Kayser-Fleischer?
Doença de Wilson
Tratamento Wilson?
Trientina (Quelante) ou transplante hepático
Diagnóstico Doença de Wilson?
Triagem: queda de ceruloplasmina
Confirma: anéis de KF/ aumento Cu urinário/Genética
Dúvida: biópsia
6H da hemocromatose herediária
Hepatomegalia/hepatocarcinoma
Heart
Hiperglicemia
Hiperpigmentação
Hipogonadismo
Hartrite
Diagnóstico hemocromatose hereditária?
TRIGEM: Ferritina (>200mcg/L) e Saturação da transferrina (>50-60%)
CONFIRMA: Genética
DÚVIDA: RM | Biópsia hepática
