Medical Home
System of health care supports and services where the child and family are comfortable, welcomed and recognized partners in the health care process. The place provides care that is accessible, comprehensive, coordinated, compassionate, family-centered, community-based, and culturally effective.
Medical Home
Prohibits discrimination and ensures equal opportunity for persons with disabilities in employment, State and local government services, public accommodations, commercial facilities, and transportation. It also mandates the establishment of TDD/telephone relay services.
ADA Act
Movement from adolescence to adulthood in all areas, including home, health care, education, and community. Process should include patient, family, and PCP. Written plan should be in place by age 14.
Transition to adult care
Allows patients to receive SSI and the associated Medicaid benefits

Children with special health care needs
States must provide this coverage for children younger than 6 with family incomes up to 133% of the federal poverty level and children 6-18 years with family income at or less than 100% FPL
Medicaid
Through the ACA, increase the current eligibility for enrollment of the FPL for all recipients which would result in many more children and parents qualifying for Medicaid coverage

Defined as a disorder that has a duration of 3 or more months and affects the functional capacity of the child or requires medical attention greater than that of a peer
