A
Long standing painless inability to pass urine - due to bladder desensitisation

Causes
A
Male - BPH
Female
- Pelvic prolapse
- Pelvic masses such as fibroids
Neurological:
- peripheral neuropathy
- upper motor neurone disease such as Multiple Sclerosis or Parkinson’s disease

Clinical Features
A
Painless urinary retention
Associated voiding LUTS
Overflow incontinence
Nocturnal enuresis
Palpable distended bladder with no tenderness

Investigations
A
Abdominal exam PSA DRE Post void bladder scan Routine bloods
High pressure - USS

Management
A
Catheterisation
TWOC (not if high pressure)
High pressure - long term catheter
Treat underlying cause

Intermittent Self Catheterisation
A
Patients are taught how to catheterise themselves at regular intervals

UTI
