Renal osteodystrophy
Ground glass, loss of lamina dura, unilocular radiolucencies (osteitis cystica fibrosa)
What causes the bony changes in renal osteodystrophy?
The lack of renal conversion of 25 hydroxycholecalciferol to 1,25 hydroxycholecalciferol (active vitamin D) leads to decreased absorption of Ca which leads to increased PTH which causes the resorption of bone to augment serum Calcium levels.
Why are hemoglobin and hematocrit decreased in ckd?
Decreased erythropoietin
Why is potassium, blood urea nitrogen, and creatinine increased in ckd?
Due to the decreased glomerular filtration rate
Why is there proteinuria in ckd?
Due to increased glomerular permeability
What kind of diet should ckd patient’s be on?
Low sodium, potassium, and protein
What common antibiotic used in head and neck infection relies minimally on renal excretion?
Clindamycin
When should dialysis patients be scheduled for surgery?
Why?
The day after dialysis.
To optimize electrolyte and fluid balance
If emergent surgery is required the day of dialysis, how long after should treatment be done?
6 hours after dialysis due to heparinization. Or you can request the nephrologist to do dialysis without heparin
