Chronic Kidney Disease Flashcards
what is the purpose of the kidneys?
Purpose: filter blood/remove waste products into urine
what are the primary causes of CKD in the US?
diabetes, hypertension, and obesity (indirect)
what does CKD contribute to?
Malnutrition, inflammation, oxidative stress, “uremic-toxins”
what is chronic kidney disease?
a gradual loss of kidney function: not being able to filter waste and excess fluids in your blood
how is CKD measured?
measured by GFR, which is glomerular filtration rate stage 1: GFR > /=90 ...normal stage 2: GFR 60-80 ...mild stage 3: GFR 30-59 ...moderate stage 4: GFR 15-29 ...severe stage 5: GFR <15 ...kidney failure
how often do patients dialyze per week?
Most patients dialyze 3 days per week for 3-4 hours…weight gain between sessions
what is the average pill burden of a dialysis patient?
Average pill burden: 19 pills/day
- Blood pressure meds
- Phosphate binders
how does dialysis affect patients work life?
< ¼ of dialysis patients are well enough to return to work
what are the benefits of dialysis patients exercising during dialysis? (intradialytic exercise)
holding them accountable/captive audience
what are the disadvantages of dialysis patients exercising during dialysis?
Concerns: limited mobility
Cycling is most feasible
Resistance training difficult
what are the benefits of dialysis patients exercising during outside dialysis hours?
Benefits: in theory, unlimited options
what are the disadvantages of dialysis patients exercising during outside dialysis hours?
compliance
Access, motivation, supervision
what are some of the limitations in the exercise literature in dialysis patients
lack in control groups, lack of holding people accountable, lack of helpful/intense exercise (lack of volume of intensity)
bad data and studies conducted badly
so…does PA work with dialysis patients
PA does work but it has to be enforced and started early…It takes many patients a year or more to change their behavior
- it must be adjusted from person to person
- also factor in diet
