Persists longer than 6 months, is irreversible, and effects functioning in one or more systems. Fluctuate between maximum functioning and then relapses. Health is labile (easily altered).
Chronic Condition
Usually reversible, short duration, often severe. Signs and symptoms: abrupt, intense, and often subside after a short period of time. May affect functioning anywhere.
Acute Condition
Decrease accommodation to near/far vision.
Presbyopia
Loss of acuity for high-frequency tones.
Presbycusis
Grieving process.

How does chronic disease affect the elderly?
The normal processes of aging may impact a chronic condition by further diminishing well-being and independence; basically make things worse. Process of aging can exasperate illness/condition.
