Isoniazid

A

Sx:
AG metabolic acidosis (increased lactate)
Seizures within 1 hr of ingestion - refractory
CNS depression
Lactic acidosis

Chronic OD: 
Peripheral neuritis
Optic neuritis
Hepatitis 
Pancreatitis 
Decreased pyridoxine (B6)

Tx:
Pyridoxine gram per gram

Viscosity - how thick
Surface tension - a “surface film” - harder to move an object through the surface
Volatility - how easily a substance can vaporize

Most dangerous for aspiration pneumonitis:

  • low viscosity
  • low surface tension
  • high volatility

Ergotamines

A

Mechanism: peripheral vasoconstriction serotoninergic effects

Sx:

  • Convulsions - h/a, spasms, miosis, phsychosis, GI
  • Gangrene - due to vasoconstriction, decreased peripheral pulse, mottled skin, burning/tingling, tissue necrosis

Disulfiram (antabuse)

A

Sx: flushing, tachycardia, diaphoresis, GI, ataxia, lethargy, seizures, HA, CP

Tx: benzos, antiemetics, IVF

  • alcohol + flagyl –> disulfiram-like reaction

Vitamins

A

Fat soluble:

  • A, D, E, K
  • Stored in fat cells - don’t need to be replenished every day
  • E = anticoagulant effects
  • D = hypercalcemia, HTN
  • A = GI, hair-loss, dizziness

Water soluble: B, C

  • B3 (niacin)
  • B6 (pyridoxime)
  • B9 (folic acid)
  • B12 (cobalamins)
