Incidence of chorioamnionitis
Commoner in prematurity
Incidence 1-5% of term pregnancies and up to 25% in preterm deliveries
Risk factors for Chorioamnioitis
Prolonged ROM Long labor Multiple vaginal exams during labor internal monitoring Bacterial vaginosis and other vaginal infections
Clinical features of chorioamnionitis
Maternal fever
maternal or fetal tachycardia
uterine tenderness
foul and purulent cervical discharge
Ix of chorioamnionitis
FBC: leukocytosis
Amniotic fluid: leukocytes or bacteria
Tx of chorioamnionitis
IV antibiotics: Ampicillin and gentamicin. Clindamycin if C/S for anaerobic
expedient delivery regardless of gestational age
Complications of Chorioamnionitis
Bacteremia of mother or fetus, wound infection if C/S, pelvic abscess, infant meningitis.
