What do statins inhibit?
HMG-CoA reductase
Describe the mechanism of action of statins
Inhibit cholesterol synthesis in liver (due to inhibition of HMG-CoA reductase)
What are the adverse drug reactions of statins?
Myopathy, increased transaminase levels, GI effects, arthralgias and headaches
What are some secondary benefits of statins?
Anti-inflammatory, plaque reduction, imroved endothelial cell function, reduced thrombotic risk
When are statins used?
Prevention of CVD for adults who have an increased risk
Given after cardiac events
Familial hypercholesterolaemia
What do fibrates do?
Reduce triglyceride production and increase HDL
SOME reduction in LDL
What are the side effects of fibrates?
GI upset, cholelithiasis, myositis, abnormal LFTs
What does nicotinic acid do?
Reduces VLDL and increases HDl
Lipid lowering effect by inhibition of lipoprotein (a) synthesis
What are the adverse effects of nicotinic acid?
Flushing, itching, headache, hepatotoxicity, activation of peptic ulcer, hyperglycaemia
What does ezetimibe do?
Selectively inhibits intestinal cholesterol absorption - decreased intestinal delivery of cholesterol to the liver
What are the adverse drug reactions of ezetimibe?
Headache, abdominal pain and diarrhoea
What are fibrates plus statins associated with an increased risk of?
Myopathy and rhabdomyolysis
What is the target cholesterol level?
Under 5 mmol/L
