Define cholesteatoma
locally destructive expansion of keratinised stratified squamous epithelium within the middle ear
Aetiology of cholesteatoma
Congenital
Acquired (secondary to attic perforation in chronic suppurative OM)
Symptoms and signs of cholesteatoma
10-20yo
98% ear discharge OR conductive hearing loss
Foul smelling white discharge
Headache, pain
CN Involvement (vertigo, deafness, facial paralysis)
suspected in any patient with unexplained unilateral ear discharge, not responsive to ABx
Investigations for cholesteatoma
Otoscopy: pearly white + surrounding inflammation)
CT (if unsure diagnosis)
management for cholesteatoma
Referral for surgery
Complications of cholesteatoma
Deafness
Meningitis
Cerebral abscess
