stage 1 - provide safety and stabilization of environment
stage 2- reduce arousal and regulate emotion through symptom reduction and memory work (find comfort, repressed memories and transform them)
stage 3- catch up on developmental and social skills, develop a value system (nurturing self awareness, coping skills, socially appropriate goals, healthy support systems)
reactive attachment disorder
rarely or minimally seeks or responds to comfort
at least 2 of the following: minimal social and emotional responsiveness, limited positive effect, episodes of unexplained irritability, sadness, or fearfulness
at least 1 of the following: neglect or deprivation, changes of primary care providers, rearing in unusual settings
behaviors of reactive attachment disorder include
withdraw, fear, sadness, irritability
sadness and listless appearance
not seeking comfort, showing no remorse
failure to smile
watching others closely
failing to ask for support
failure to reach out when picked up
no interest in playing peekaboo or other interactive games
disinhibited social engagement disorder
occurs first 2 years of life
child approaches and interacts with unfamiliar adults with 2 of the following
reduced reservation about approaching unfamiliar adults
overly familiar and violates social/cultural boundaries
doesn’t check back with caregiver
willing to go with unfamiliar person without reservation
evidence of severe social neglect
caregiver neglects Childs basic physical and emotional needs
repeated changes of caregiver
treatment of reactive attachment and disinhibited social engagement
always include the primary familial caregivers
ensure that child experiences positive interactions with caregivers and staff
experiences attachment through 5 senses
has a safe stable living situation that is nurturing, responsive and caring
provide consistent care giver
provides positive, stimulating and interactive environment
addressing the childs medical, safety and housing needs
hug, hold, touch, feed and talk to child
use story telling
encourage meals with other children and familial caregivers
adjustment disorder
happens within 3 months of exposure to stressor (retirement, divorce, new sibling, break-up)
affects ability to function
reaction is out of proportion
symptoms END by 6 months (anxiety, depression, regressive behaviors, fearful or acting out behavior)
Requires support, understanding, encouragement
active listening, therapeutic communication skills
assist in increasing coping skills
