- Over 65 years old
- Head trauma

- Already taken other sedatives (e.g., benzodiazepines, alcohol, etc.)

- Atropine 0.5mg IV/IO
may repeat in 3 mins, max total dose 3mg

After Ketamine if pt begans to wake up
A
- Versed 5mg IV/IO/IM/IN
may repeat x1 in 5 minutes

Rapid cooling for a tempeature >103 F
A
- Cold NS 1L
- Sodium bicarbonate 100mEq over 2 mins

A
Ketamine

Treatment for pain managment
A
- Fentanyl 100mcg IV/IO/IM/IN
may repeat x2 in 5 mins

For severe Pain management 7 or higher
A
- Ketamine
25mg in 50mL administer IV/IO utilizing a 60 gtt set, run wide open.
-Reassess pain scale after half of the infusion has been administer. Continue infusion if needed, may repeat x2

If unable to establish IV with Ketamine
A
IN/IM 25mg
may repeat x2, in 5 mins

A
- Lidocaine 40mg IO over 2 mins
-Allow Lido to dwell in IO space for 1 minute
flush with normal saline
may repeat 20mg IO X1

PEDIATRIC: Pain management
A
- FENTANYL 1mcg/kg IV/IO, over 2 minutes
may repeat x1 in 5 minutes
max total dose 200mcg

PEDIATRIC: If unable to give Fentanyl IV

- IN/IM 1.5mcg/kg

PEDIATRIC: Severe pain 7 or higher

- KETAMINE ( ≥ 3 years old): 1mg/kg IN/IM
