13
Q

What is the purpose of a carotid endarterectomy in a client who has had a transient ischemic attack?

A

Prevent a future stroke by removing an atherosclerotic plaque or thrombus blocking flow to the brain.

Q

In an ischemic stroke, what is the area of low cerebral blood flow surrounding tissue infarction during early ischemic cascade?

A

The Penumbra region, which can be salvaged if intervention is timely.

17
Q

What are priority interventions for a client with increased ICP to prevent increased blood pressure due to anxiety and/or activity?

A

Place in a quiet, dark, nonstimulating environment, elevate the HOB to 30 degrees, and keep the head neutral. Avoid flexion, extension, and turning the head from side to side.

18
Q

What effective positioning is needed after an ischemic stroke to promote hyperextension of the hip joints to normalize gait and prevent knee and hip flexion contractures?

A

In a prone position for 150-30 minutes several times a day, place a pillow under the pelvis from the umbilicus to the upper third of the thigh.

19
Q

What are some risks for a client experiencing increased ICP?

A

Seizures and HYPOnatremia.

20
Q

A priority goal for a client experiencing a stroke?

A

Maintain and improve cerebral tissue perfusion.

21
Q

What is the most serious risk factor for a repeat hemorrhagic stroke?

A

Hypertension: educate the patient on taking antihypertensives as prescribed.

22
Q

What is the main contraindicated treatment for someone with a hemorrhagic stroke, and why?

A

Thrombolytic therapy because it increases bleeding.

23
Q

From what side should you approach a stroke client if they have vision loss, and where should you place items they may need?

A

The unaffected visual and physical side.

24
Q

What education is needed for a client experiencing one-sided visual loss?

A

To compensate, turn the head toward the direction of the vision loss and remind the client regularly of the other side of the body.

25
Q

What kind of exercise is needed for a client who has recently had a stroke?

A

Passive range of motion of affected extremities four or five times daily to prevent contracture and deterioration of the neuromuscular system.

26
Q

A client has homonymous hemianopsia after a stroke. What is it called when the client neglects the side of the body they can no longer see, and what are the nurse’s interventions?

A

This is called amorphosynthesis. The nurse should continually remind the patient about the affected side and place extremities where the client can see them.

27
Q

What are potential interventions and treatments for muscle spasms after a stroke?

A

Stretching and medication such as baclofen and tizanidine. A temporary fix of 2-4 months may be reached through a botulism A injection.

28
Q

What medication is best for post-stroke pain and depression?

A

Amitriptyline.

29
Q

What are the best interventions for shoulder pain after a stroke?

A

Corticosteroids, heat therapy, acupuncture, and forward shoulder stretch with hands interlaced.

30
Q

When should ambulation occur after an ischemic stroke?

A

As soon as physically possible.

31
Q

Where do hemorrhagic strokes take place?

A

In the brain or subarachnoid space.

32
Q

Where do ischemic strokes take place?

A

Vascular occlusion.

33
Q

Other names for an ischemic stroke?

A

Thrombotic or lacunar.

34
Q

COVID-19 increases the risk for what type of stroke?

A

Ischemic stroke due to increased risk for blood clotting abnormalities.

35
Q

What arrhythmia is most commonly responsible for cardiogenic embolic strokes?

A

Atrial fibrillation.

36
Q

List the steps of an ischemic cascade in order.

A
  1. Central blood flow decreases to less than 25mL per 100g blood per minute.
  2. Mitochondria lose energy and switch to anaerobic respiration.
  3. Lactic acid is then generated
  4. This leads to Ph changes
  5. Membrane pumps controlling electrolytes fail.
  6. Cells cease to function.
37
Q

What are nursing interventions immediately after an ischemic stroke?

A

Reduce ICP by administering an osmotic diuretic like mannitol and positioning the patient’s HOB at 30 degrees. Handle the patient’s secretions to avoid aspiration. Keep PaCO2 in an acceptable range. Do not start mobilization until after ICP threat has passed.

38
Q

What are the most important things to monitor in a patient receiving t-PA therapy?

A

Bleeding, cardiac status, and neurological status.

39
Q

What are necessary interventions for a patient experiencing dysphagia after a stroke?

A

A swallowing assessment should be performed within 4-24 hours after a stroke. Educate the client to take smaller boluses of food and eat food that is easier to swallow. They may be started on thick liquids or a pureed diet. Have the client sit upright, preferably in a chair, and instruct them to tuck the chin forward when swallowing.

40
Q

What is a potential complication of stroke due to dysphagia, and possible interventions?

A

Aspiration leading to pneumonia. Deep breathing and coughing, small and frequent meals, and ambulate asap.

41
Q

What are the best predictors of outcome during a stroke?

A

NIHSS score, LOC at the time of admission, and the patient’s age.
