Injurious or unprofessional actions that harm another

Leaving a pt without care or without properly transferring care

Confidential document that describes any patient accident while the person is on the premises of the health care agency

medical errors that are preventable such as falls, uti’s from catheters, and pressure ulcers (my ulcer!)

End of life directions for a patient who may not be able to give instructions regarding their health and well-being such as a DNR
