Understanding the needs of a population in a certain area and factors that influence the health promotion and maintenance of that population.
Public Health Nursing
Nursing care provided in the community with a focus on groups, families, and individuals in the community. This type of nurse may also specialize in working with a particular subpopulation within the community. Often work with difficult systems like welfare and encourage them to be more responsive to the needs of the population.
Community Health Nursing
The first level of contact between members of a community and the health care delivery system.
Community based nursing centers
Groups of patients who are more likely to develop health problems as a result of excess risks, limited access to health care services, and dependency on others for care
Vulnerable populations
The identification and reporting of factors that influence the health of a community i.e. new infections, adolescent pregnancy rates, and MVA’s by teenage drivers.
Incidence Rates
Making an appropriate plan of care based on assessment of patients and families and coordinating needed resources and services for a patient’s well-being across a continuum of care
Case Management
Structure (locale), the people (population), and the social systems
