Define a competent person
A person capable of identifying existing and predictable hazards.
Where should flaggers be located?
Flaggers should be positioned 100 feet in front of the work zone.
Flagger sign should be placed 500 feet minimum.
What is a confined space defined as?
Large enough that an employee can enter
Limited or restricted means for entry or exit
Not designed for continuous occupancy
What are the classifications of confined space
Permitted
Non-permitted
Define a hazardous atmosphere
Atmosphere that may expose employees to risk of death, incapacitation, impairment of ability to self rescue.
Define oxygen deficiency
Less than 19.5% oxygen levels
Define oxygen enrichment
Greater than 23.5 percent oxygen by volume.
Define methane gas.
Colorless, odorless and tasteless. The major flammable gas in natural gas pipelines. 55% as heavy as air.
Define toxicants.
Substance that can act as poisons.
Define a confined space
Any manhole, sewer, diversion structure, pump station, underground metering vault.

What should atmospheric analyzer contain?
Audible and Visable alarm that will warn when flammable gasses exceed 10 % of the LEL or when oxygen drops below 19.5% or rises above 23.5%.
Atmospheric testing equipment is just one tool and should not be considered the ultimate test for the presence of explosive gasses.
They should not preclude the use of ventilation of some sort in the confined space. Ventilate for 10 minutes prior to entry.
When should atmospheric testing stop?
Atmospheric testing must continue while space is occupied.
How many operators should be present for manhole entry.

How far should spoil be set back from the excavated area.
2 feet.
Define shoring.
A complete framework of wood or metal designed to support the walls of a trench.
Define sheeting.
Sheeting is the solid material placed directly against the side of the trench. Fully dimensional rough cut timber.
Who should work on electricity.
The first rule that applies to electrical systems: if you are not qualified, do NOT, under any circumstances, attempt to work on electrical systems.
Define a tailgate safety meeting.
Brief safety meetings held every 7 to 10 days.
Define lock out / tag out.
OSHA standards require that all equipment that could unexpectedly start up or release stored energy be locked and tagged whenever it is being worked on.
Define Safety Data Sheets. SDS.
