Family History
Myocardial Infarction, coronary revascularization, or sudden death before 55 year in father or other male first degree relative
or 65 year in mother or other female first degree relative
Cigarette Smoking
current cigarette smoker or those who quit within the previous 6 months or exposure to environmental tobacco smoke
Physical Inactivity
not meeting the minimum of 500-1000 Met/min or 75-150 min/week of moderate to vigorous PA
BMI/Waist Circumference
BMI: >/=30 kg
Waist Girth >102 cm (40 in) in men or >88 cm (35 in) in women
Blood Pressure
Systolic Blood Pressure: >/= 130
Diastolic: >/= 80
or on antihypertensive meds
Lipids
LDL-C >/= 130 or
HDL-C <40 in men <50 women
non HDL-C >/= 160
or on lipid lowering medication
Blood Glucose
Fasting Blood Glucose >/= 100
2 hour plasma glucose in OGTT >/= 140
A1C >/=5.7%
HDL-C
