Chapter 13: Post-Resuscitation Care Flashcards
1
Q
What is post-cardiac arrest syndrome?
A
- Post-cardiac arrest brain injury
- Post-cardiac arrest myocardial dysfunction
- Systemic ischaemia / reperfusion response
- Persistent precipitating pathology
2
Q
Main steps in Adult Post-Resuscitation Care:
A
- Airway & Breathing
- Circulation
- Control temp
- Likely cardiac cause?
- Cause for cardiac arrest identified?
- Admit to ICU
- ICU Mx
- Secondary prevention
3
Q
What does Mx of Airway & Breathing include:
A
- Maintain sats 94-98%
- Insert advanced airway
- Capnography
- Maintain normocapnia
4
Q
What does Mx of Circulation include:
A
- 12 lead ECG
- IV access
- Crystalloid fluids
- Restore normovolaemia
- SBP > 100
- Inotropes / vasopressors to maintain SBP
- Intra-arterial BP monitoring
5
Q
What does Temperature control entail:
A
- Prevent fever
- Sedation
- Control shivering
6
Q
Likely Cardiac cause - NO - Options & Pathway:
A
- CTB or CTPA
- Rx non-cardiac cause of arrest
- Admit to ICU
7
Q
Likely Cardiac cause - YES - Options & Pathway:
A
- 12 lead ECG
- Check for ST elevation
- If ST elevation IS present - Cor Angio +/- PCI
- If ST elevation NOT present - Consider Cor Angio +/- PCI
8
Q
Cause for arrest identified - YES - Pathway:
A
Admit ICU
ICU Mx:
1. Temp control - prevent fever for at least 72 hrs
2. Maintain normoxia
3. Maintain normocapnia
4. Maintain normoglycaemia
5. Avoid hypotension
6. Echo
7. Dx / Rx seizures - EEG, sedation, AEDs
8. Delay prognostication for at least 72 hrs
9
Q
What does Secondary Prevention entail?
A
- ICD
- Risk factor Mx
- Screen for inherited disorders
