- establish questionnaire flow and layout
- evaluate the questionnaire

- prepare final copy
- implement the survey

CRITERIA FOR GOOD QUESTIONNAIRE
A
Does it provide the necessary information?
does it consider the respondent? make sure its not too wordy, confusing or designed poorly.
does it meet editing, coding and data processing requirements?
