What are some examples of scenarios where you should enact SMR?
- Unresponsive patient and you do not know MOI
- A fall from a height greater than 1 meter/5 stairs
- MV collisions or ejection from a MV (motor vehicle)
- Any injury where the patient’s helmet is broken
- Injuries involving severe blunt trauma to head or trunk
- Any deeply penetrating wound to the head or trunk
- Any diving mishap
- In general any high energy impacts/injuries
What are some S/S of dyspnea?
Inadequate rise and fall of the chest
High degree of effort on respirations
Very fast or very slow respiration rates
What are agonal respirations?
Snorting/gurgling/gasping sounds - breathing often associated with cardiac arrest and is NOT adequate to support life
When should you administer a nasal cannula and what flow rate should you set with it?
91-94% SpO2 (Mild hypoxia) - flow rate of 1-4 LPM
When should you administer a standard oxygen mask and what flow rate should you set with it?
91-94%, 6-10 LPM
When should you administer a non-rebreather and what flow rate should you set with it?
90% or lower SpO2 (moderate hypoxia) with flow rate of 10+ LPM - if you are a SOLO responder
When should you administer a BVM and what flow rate should you set with it?
90% or lower SpO2 and a flow rate of 15-20 LPM - if you have another person to do the BVM with
What does OPQRST stand for?
O - Onset - When did it begin?
P - Provocation - What causes the pain to get worse?
Q - Quality - what does the pain feel like (sharp, dull, stabbing, etc)
R - Region/Radiation - Where does it hurt and does it radiate to other areas?
S - Severity - How bad is the pain? Scale of 1-10
What are the different stages of hypoxia and the associated SpO2% levels?
95-100 - normal oxygen content in the blood
91-94 - mild hypoxia
86-90 - moderate hypoxia
85% or lower - severe hypoxia
When should reassessments take place?
Immediately following RBS. After that, every 5 minutes for an unstable patient and 15 for a stable one
What age range is a neonatal, and what are the normal vital signs?
Up to 28 days old is a neonate.
Pulse 120-160
BP 80/40
Respirations 40-60
What age range is an infant, and what are the normal vital signs?
1-12 months
BP 80/40
Pulse 100-120
Respirations 30-40
What age range is a child , and what are the normal vital signs?
1-8 years old
BP - 90/50
Pulse - 80-120
Respirations - 16-24
