Chap 2 - Endocrinology and Metabolism Flashcards
Galactorrhea without evidence of hCG
Prolactinoma - measure serum prolactin
Classic physical exam finding in prolactinoma
Bitemporal hemianopia
Decreased cortisol release sequelae
N/V, abdominal pain, hyponatremia, hypokalemia, hypotension in setting of infection
Treat hypotension with IV hydrocortisone
Prolonged prednisone course without taper
Secondary adrenal insufficiency => hypotension => IV cortisol
Shock picture with sellar mass
Pituitary tumor apoplexy => glucocorticoid administration to manage hypotension 2/2 acute ACTH depletion
What is central hypothyroidism?
Pituitary does not make TRH => low TSH => cannot use TSH to measure effect of levothyroxine => must increase levothyroxine until T4 is normalized
Hashimoto thyroiditis treatment?
Levothyroxine, not liothyronine (T3 analogue)
Forms of destructive thyroiditis
Subacute (painful), silent, post-partum
