Ch.2 application of pharmacology in nursing practice Flashcards
6 rights to meds
- drug
- patient
- dose
- route
- time
- documentation
nurses roles:
observe drug responses: ** need to know what the drugs do and their side effects to anticipate responses
mistakes- allergies, drug interactions, contraindications, appropriate dose/route
ethical points:
- it is ethically and legally wrong to administer a drug that is harmful to a patient
- WE are patient advocates
8 aspects: application of pharmacology
guidelines for correct drug administration:
- read order carefully and verify
- read med labels carefully (identity, amount)
- verify dosage calculations
- verify identity of patient
- implement special handling
- DO NOT administer if you do not understand reason for use
Patient Education :
Dosage and administration:
- name of med (generic/trade)
- dosage size and schedule of administration
- technique (route, preparation)
- duration of drug use
- storage (refrig)
Promoting therapeutic effects:
- improves adherence (if pt understands what to expect, onset, effects
Minimize adverse effects:
-educate patient
minimized adverse interactions:
- dangers of drug-drug interactions, foods to avoid, herbs, etc
application of nursing process in drug therapy: ADPIE
ADPIE
Assessment:
- collect data (subjective/objective)
- provides foundation for subsequent steps
- methods (interview, med/drug hx, physical exam, labs)
Diagnosis
- analyze database to determine actual/potential problem
- form nursing dx
Planning:
- interventions that solve or prevent the problem identified
- must be individualized
- ongoing process that constantly changes
Implement:
- carry out intervention and record outcomes
Evaluation:
- determines degree which tx has been successful
- analyze data obtained during implementation
