2. Listing of the hazardous material in accordance with the HMIS
3. List of hazardous material in excess of 1% of contents
4. List of each chemical in excess of 5% of contents
5. Info as to what is hazardous about the agent according to the MSDS
6. Manufacturer’s or service agency’s name, address, and phone number
