Magnet Recognition

Health/medical care that takes place in the patients home
Patient Centered Medical Home
Service that provides short-term relief or time off for people providing home care to an ill, disabled, or frail older adult
Respite Care
Assessment of patient outcomes based on nursing care; Changes in the pts’ symptoms, function, safety, psychological state, and cost as a result of nursing intervention
Nursing-sensitive outcomes
Care in which patients who have sx/s of a disease are diagnosed and treated
Acute Care

Assisted Living

Evidence based practice
Provides intermediate medical, nursing, or custodial care for patients recovering from acute illness or pts with chronic illnesses or disabilities
Extended care facility
Care that allows pts to live and remain at home with comfort, independence, and dignity while alleviating the strains caused by terminal illness
Hospice
System where payer has control over primary healthcare services delivery for a defined patient population
