Mechanism of Injury
The forces of energy transmission applied to the body that cause injury.
Nature of Illness
The general type of illness the patient is experiencing
Pertinent Negatives
Signs and symptoms that is important to note the patient does not have
Chief Complaint
The most serious thing the patient is concerned about
Priapism
A painful persistent erection caused by spinal cord injury, erectile dysfunction drugs, and sickle cell disease
Palpate
To feel
Auscultate
To listen using a stethoscope
Jaundice
Yellowing of the skin caused by liver dysfunction
Diaphoresis
Pale, clammy skin
Primary problem
The most serious thing the EMT is concerned about
Signs
Objective findings that can be verified by the EMT
Symptoms
Subjective findings that the patient feels but cant be verified by the EMT
Paradoxical motion
When one section of the chest rises on inspiration while another area of the chest falls
Subcutaneous emphysema
Pockets of air accumulating inside the chest but outside of the lungs
Guarding
Involuntary contraction of the muscle of the abdomen in an effort to minimize pain
SAMPLE
S: signs and symptoms
A: allergies
M: medications
P: past medical history
L: Last oral intake, what the patient ate and when
E: Events leading up to the present complaint
OPQRST
O: onset
P: provocation
Q: quality
R: region or radiation
S: severity
T: time
PASTE
P: provocation, what makes the dyspnea worse
A: associated signs and symptoms
S: sputum, is the patient coughing? If so are they coughing up sputum? If so what color?
T: time, temperature
E: Exacerbation, is the dyspnea getting worse over time
