Community-Based Practice
Broad range of health related services: Prevention and health promotion, acute and chronic medical care, habilitation and rehabilitation, and direct and indirect service provision
Goal of CBP?
For client and practitioner to become integral parts of the community
Community Health Promotion
any combo of educational and social supports for people taking greater control of, and improving their own health or the health of a geographically defined area
Educational programs can be directed towards ind, fam, grps, or communities through schools, worksites, organizations, and or mass media
Community-level interventions
- “attempt to modify the socio-cultural, political, economic and environmental context of the community to achieve health goals”
- Population-based approaches
- Community-level interventions are directed at impacting systems that affect health in communities.
Community-centered initiatives/interventions
- generated by leaders and members of the community and typically utilize existing community resources
- promote community participation, exchange of information, and community autonomy. The role of the professional is as a consultant, facilitator, and mentor in the community.
Dynamical Systems Approach
- recognizes the complexity of the social history of health and provides a framework for assessment and intervention at various levels of systems, including individual, interpersonal, organizational, community, and public policy levels
- “a chain reaction of adaptations and adjustments is created in other parts of the system”
Ecological approach
- considers client embedded in and interacting with a variety of environments and contexts.
- requires practitioner to consider both client’s capabilities & constraints and the environmental enablers & barriers
