Capital extension muscles
The suboccipitals, Longissimus capitis, Splenius Capitis, Semispinalis capitis, Spinalis capit
Capital extension Grade 3, 4, 5 position
Patient : Prone with head off the end of the table and arms at side.
PT : ask the patient to tilt the head up and look at the wall. Place resistance over the occiput straight down.
Capital extension Grade 2, 1, 0
Supine with head on table and arms out at the side.
PT cue: ask patient to tilt chin upward
Capital extension Palpation
palpate the suboccipital muscles
Cervical extension MMT muscles
Longissimus cervicis, Semispinalis cervicis, Iliocostalis cervicis, Splenius cervicis, Traps, Spinalis cervicis
Cervical Extension grade 5,4,3 postion
Patient: Prone with head off the table and arms at the side
PT: have the patient lift head while still looking at the floor. Apply resistance at the parieto-occipital area straight down
cervical extension grade 2,1,o position
Supine with head fully supported by the table. Arms at sides.
PT: have hands under the patients head and have the patient push their heads into PTs hands
Cervical extension MMT palpation
palpate behind the neck
Capital flexion (chin tuck) MMT muscles
Rectus Capitis Anterior, Rectus Capitis Lateralis, longus capitis
capital flexion (chin tuck) MMT grade 5,4,3 position
Supine with head on table arms at side. Ask patent to chin tuck and add resistance to either the forehead or mandible
capital flexion (chin tuck) MMT grade 2,1,0 position
Supine with head supported on table and arms at side. Partial range is a 2
capital flexion (chin tuck) MMT palpation
not able to palpate unless there is severe atrophy
Cervical Flexion MMT muscles
SCM, Longus colli, Scalenus anterior
Cervical Flexion MMT grade 5,4,3 position
Supine with knees bent and feet on the table with arms at the side. Have the patient lift head from the table while keeping a chin tuck and eyes toward the ceiling. Resistance on the mandible or the forehead
Cervical Flexion MMT 0,1,2 position
Supine with head supported in table with arms at sides. Patient rolls head from side to side to test the SCMS
