Cervical OSCE Flashcards
What is the Compression Test?
Head/neck in neutral, load downward
+: UE pain, paresthesia, numbness
What is the Spurling’s Test?
Test: Nerve root compression
Tested with axial force 1) neutral 2) extension 3) SB/Rotation toward or away
+: reproduction of pain, paresthesia in Nerve root distribution
INDICATES: Central neuropathy
What is the Neck Distraction test?
Hand under chin, aroudn occiput, lifts upwards
+: Alleviation of symptoms
INDICATES: Central neuropathy
What is Thoracic outlet syndrome? what are the presenting symptoms?
result from compression of NV bundle
Distal pulses decreased/absent, digital cyanosis or ischemia
Sympathetic autonomic dysfunction, UE symptoms
What is Adson’s test
Test: Locate radial pulse, ABducts, extends, ER shoulder, PTs head extended and rotated toward affected side, then EXT and rotated away from affected side
+: loss or change in pulse, pain/paresthesia
INDICATES: Thoracic outlet syndrome (compression of subclavian artery between scalenes (when looking away form affected side) or 1st rib/cervical rib (when looking towards)
REMEMBER: When looking TOWARDS the BBQ RIBS, your heart stops beating (excitement!) and you look AWAY from the SCALE (because, yikes, too much ice cream for Emma)
Roos Test or EAST Test
Touchdown symbol (arm abducted, ER, elbow flexed) open and close fist for 3 minutes
+: reproduction of symptoms pain/paresthesia
INDICATES: Thoracic outlet syndrome
Nuchal Rigidity
TEST: lifted head up
+: MARKED NECK STIFNESS
INDICATES: Inflam in subarachnoid space (menigitis or subarach hemorrhage)
Brudzinski’s Sign
TEST: lift head up
+: flexion in both hips and knees
INDICATES: Inflam in subarachnoid space (menigitis or subarach hemorrhage)
Kernig
TEST: supine, flex hip and knee to 90
+:increased resistance to EXT and pain behind knee
INDICATES: Meningeal/dural irritation
What is a Jefferson fracture?
axial compresion, C1 fracture
What is a wedge fracture?
flexion and compression injury (body of SC looks dented)
