What is the management of cervical cancer in a pregnant patient who desires termination?
Up to stage IB - rad hyst, lymph nodes, fetus left in situ
IIB or greater - chemoradiation, brachytherapy, termination
What is the management of cervical cancer in a pregnant patient who desires to continue a pregnancy?
Neoadjuvant chemotherapy
Delivery via classical cesarean section
Usual treatment
Stage 1 cervical cancer
Confined to the uterus
1A - microscopic disease, < 5 mm depth
1B - macroscopic disease or >= 5 mm depth
Stage 2 cervical cancer
Extension beyond the uterus
2A - upper 2/3 of vagina, no parametrial invasion
2B - parametrial invasion
Stage 3 cervical cancer
3A - lower third of the vagina
3B - pelvic sidewall or causing hydronephrosis
3C - involvement of pelvic or paraaortic lymph nodes
Stage 4 cervical cancer
Invasion of mucosa of bladder or bowel or extension outside of the pelvis
4A - adjacent pelvic organs
4B - distant organs
