Sitting
First stage: compression with head in neutral. Second stage: compression with head in extension. Third stage: head in extension/rotation to unaffected side, then side of complaint, with compression. If symptoms are produced at any point, do not proceed to next stage
Foraminal Compression
Cervical radiculitis
Pain radiates in arm in direction head is side flexed during compression.
Sitting
Slowly lift the head, applying traction to the cervical spine.
Distraction Test
Nerve root compression
Relieving or decreasing the pain when head is lifted or distracted.
Sitting or supine
Passively (or patient actively) elevate arm through abduction, so hand or forearm is resting on top of the head
Shoulder Abduction Test
Cervical extradural compression
A decrease in or relief of symptoms
Sitting
Patient takes deep breath and holds it while bearing down, mimicking a bowl movement.
Valsalva Test
Increased pressure on spinal cord
Increased pain
Long sitting
Passively flex head and one hip simultaneously, with leg kept straight
Lhermitte’s Sign
Dural/meningeal irritation in the spine
Possible cervical myelopathy
Sharp pain down spine and in upper or lower limbs.
Supine
Passively take head and neck into extension and side flexion. Rotate neck to the same side and hold for about 30 seconds
Vertebral Artery Test
Problem of the vertebrobasilar artery
Creates symptoms when opposite artery is affected.
Can include dizziness, visual disturbances, disorientation, etc.
