DDX Thunderclap

A

Most common

  • Subarachnoid hemorrhage
  • Reversible cerebral vasoconstriction syndromes (RCVS)

Conditions that less commonly cause thunderclap headache:

  • Cerebral infection (eg, meningitis, acute complicated sinusitis)
  • Cerebral venous thrombosis
  • Cervical artery dissection
  • Spontaneous intracranial hypotension
  • Acute hypertensive crisis
  • Posterior reversible leukoencephalopathy syndrome (PRES)
  • Intracerebral hemorrhage
  • Ischemic stroke

Conditions that uncommonly or rarely cause thunderclap headache:

  • Pituitary apoplexy
  • Colloid cyst of the third ventricle
  • Aortic arch dissection
  • Aqueductal stenosis
  • Brain tumor
  • Giant cell arteritis
  • Pheochromocytoma
  • Pneumocephalus
  • Retroclival hematoma
  • Spinal epidural hematoma
  • Varicella zoster virus vasculopathy
  • Vogt-Koyanagi-Harada syndrome

  • tx : Carbamazépine (tégrétol), Oxcarbamazépine (trileptal, SIADH +++), Leslicarmabazépine Gabapentin (neurontin)

traitement migraine

A
  1. diminuer triggers (ROH, sommeil, alimentation, fatigue, chaleur, lumière, écrans, hormones)
  2. AINS (cambia, voltaren, Anaprox)
  3. Triptans / ergotamine (max 10x/mois, 2x/jours. ne pas redonner si première dose aps efficace. action sur R 5HT 1F-1D = vasoconstriction)
  4. Anti CGPR : méfiance HTA, pas d’ES,
  5. Botox : efficacité après 2-3 doses et faut attendre 3 mois par doses
  6. prophylaxie : BBloc (pronolol, Nadolol, Metoprolol) / AED (topamax, épival, neurotin) / Antidepresseurs (élavil, effexor)
