invasion of dermis and subcutaneous fat

strep. pyogenes +/- staph. aureus (surgical wound sites), beta-haemolytic streps

A
diabetics

how does it clinically present?
A
generalised swelling seen in legs
macular hot erythema with ill defined margins that is often spreading
fevers, rigours and nausea

what investigations are used?
A
usually clinical diagnosis
atypical = blood cultures

how is it managed?
A
antibiotics
rest
elevation
analgesia
splint

what antibiotic is used and what is the alternative if allergic?
A
flucloxacillin
doxycycline
