• anti-tissue transglutaminase Antibodies.
• anti- deamidated gliadin peptide antibodies.
• IGA anti- endomysial .

Duodenal biopsy
- flattening of the mucosa with loss of villi • hyperplasia of crypts
- chronic iflammatory infiltration of the lamina propria with lymphocytes.
- the most changes are Seen in the duodenum and proximal jejunum.
Clinical presentation of Celiac disease ?

hypersensitivity to gluten, a protein
found in wheat. (Gliadin)
- Children • abdominal distension • diarrhea • failure to thrive !!!!
Adults
• chronic diarrhea and bloating.
•• dermatitis herpetiformis !!
Due to lgA deposition at the tips of dermal papillae // resolves with gluten-
free diet.
•• iron deficiency anemia
Diagnosis of lgA deficiency ?
igG antibodies are also present and are useful for diagnosis in individuals with igA deficiency
*** increased incidence of lgA deficiency is seen in celiac disease
Late Complication of celiac disease in refractory disease despite good dietary control ??

- Small bowel carcinoma
- T-cell lymphoma.
Tropical sprue

- • Unknown organism. • Tropical regions • arises after infections diarrhea • responds مستجيب to antibiotics// tetracycline.
Associated with :
•megaloblastic anemia due to folate deficiency.
• later B12 deficiency !!
