CDAD Flashcards
- Overview of CDAD - Treatment
1
Q
CDAD
A
- Clostridium difficult associated disease
- Gram positive
2
Q
About
A
Major identified cause of nosocomial diarrhea associated with antibiotic therapy
- toxigenic strains: synergistic activity of toxins A & B causes extensive damage to colonic mucosa
3
Q
Treatments
A
- Discontinuation of current antibiotic
- Specific antibiotic treatment
a) Metronidazole Antibiotic Treatment: first line agent
b) Vancomycin: second line agent
4
Q
Metronidazole Antibiotic Treatment
A
- First line agent
- Nitroaryl compound
- Prodrug activated by anaerobic organisms by: The reduction of the nitro group to hydroxyl amine group
- During reduction: reactive species produced can damage bacteria cell components (DNA, pens, membranes)
5
Q
Advantages of MAT
A
Prevent selection of vancomycin resistant bacteria
Reserve vancomycin use for the treatment of MRSA infections
6
Q
Vancomycin
A
- Second line agent
- Prescribed instead of metronidazole if:
Failure to respond to antibiotic 3-5 after
Pregnancy and lactation
Intolerance to metronidazole
Critically ill patients
Patients resistant to metronidazole
