Aim
Challenge irrational beliefs and reality test them by grounding in society reducing distress which can make symptoms worse. Use positive self talk to deal with it e.g. ignore auditory hallucinations.
Cognitive Restructuring
ABCDE Framework - identifies activating event, explain irrational beliefs, recognise consequences, dispute beliefs, results in restructuring of beliefs
Copying Strategy Enhancement
Type of CBT. Aims to develop and apply at least 2 effective copying strategies for each psychotic symptoms and accompanying stress. Strategies may have already been developed by patient as often can identify trigger so manage stress (Tarrier)
Strategies
cognitive ones - distraction, concentrating on certain tasks, positive self-talk. behavioural - relaxation techniques, social withdrawal/increasing social contact, loud music to drone out voices, behavioural experiments. Given homework to consolidate strategies.
Two parts to therapy
First develop rapport with patient and identify trigger of psychotic symptom and review existing ones and develop copying strategies. Next target specific symptoms and find strategies to deal with them directly.
Strength 1
No negative side effects. Doesn’t have side effects unlike drugs. However, end treatment early due to length and engagement. Have to take drugs to make CBT accessible.
Strength 2
Supportive evidence. Senesky et al (2000) reduction of symptoms improving after 9 months stopped. However, CBT costly and long waiting lists. Drug treatment better option
Strength 3
Evidential support. Jauhar et al (2014) CBT had significant effect. However, small significance.
