Medical Clearance Recommended; Following Medical Clearance, Light to Moderate Intensity Exercise Recommended (May gradually progress as tolerated following ACSM Guidelines)
If participates in Regular Exercise and has No CV, Metabolic or Renal Disease and no signs or symptoms suggestive of these…
Medical Clearance Not Necessary; Continue Moderate or Vigorous Intensity Exercise (May gradually progress following ACSM Guidelines)
If participates in Regular Exercise and has Known CV, Metabolic or Renal Disease and Asymptomatic…
Medical Clearance for Moderate Intensity Exercise not Necessary; Medical Clearance (within the last 12 months if no change in signs/symptoms) Recommended before Engaging in Vigorous Intensity Exercise; Continue with Moderate Intensity Exercise (Following medical clearance, may gradually progress as tolerated following ACSM Guidelines)
If participates in Regular Exercise and has Any signs or symptoms suggestive of CV, Metabolic or Renal Disease (Regardless of disease status)..
Discontinue Exercise and Seek Medical Clearance; May return to Exercise Following Medical Clearance (Gradually progress as tolerated following ACSM Guidelines)
Exercise Participation
Performing planned, structured physical activity at least 30 minutes at moderate intensity on at least 3 days a week for at least the last 3 months
Light Intensity Exercise
30-39% HRR or VO2R; 2-2.9 METs; RPE 9-11; an intensity that causes slight increase in HR and breathing
Moderate Intensity Exercise
40-59% HRR or VO2R; 3-5.9 METs; RPE 12-13; an intensity that causes noticeable increases in HR and breathing
Vigorous Intensity Exercise
> /= 60% HRR or VO2R; >/= 6 METs; RPE >/= 14; an intensity that causes substantial increases in HR and breathing
Cardiovascular (CV) Disease
Cardiac, Peripheral Vascular, or Cerebrovascular disease
Metabolic Disease
Type 1 and Type 2 Diabetes Mellitus or Renal Disease
Signs and Symptoms
At rest or during activity includes pain, discomfort in the chest, neck, jaw, are, or other areas that may result from ischemia; shortness breath at rest or with mild exertion; Dizziness or Syncope: Orthopnea or Paroxysmal Nocturnal Dyspnea; Ankle Edema; Palpitations or Tachycardia; Intermittent Claudication; Known Heart Murmur; Unusual Fatigue or Shortness of Breath with Unusual Activities
Medical Clearance ACSM Guidelines
Approval from a health care professional to engage in exercise (See ACSM’S Guidelines for ETP)
Atherosclerosis Cardiovascular Disease (CVD) Rusk Factors
Age, Family History, Cigarette Smoking, Physical Inactivity, Obesity, Hypertension, Dyslipidemia, Diabetes, HDL-C
Age
Men >/= 45yr, Women >/= 55yr
Family History
Myocardial Infarction, Coronary Revascularization or Sudden death before 55yr in father or other male degree relative or before 65yr in mother or other female first degree relative
Cigarette Smoking
Current cigarette smoker or those who quit within the previous 6 months or exposure to environmental tobacco smoke
Physical Inactivity
Not participating in at least 30 minutes of moderate intensity physical activity (40-59 VO2R) on at at least 3 days of the week for at least 3 months
Obesity
BMI >/=30 kg*m2; waist girth >102cm (40in) for men and >88cm (35in) for women
Hypertension
Systolic blood pressure >/= 140mm Hg and /or Diastolic >/= 90mm Hg, confirmed by measurements in at least two separate occasions, or on antihypertensive medication
Dyslipidemia
Low-density lipoprotein cholesterol >/= 130 mgdL (3.37mmolL) or High-density lipoprotein cholesterol <40 mgdL (1.04mmolL) or on lipid-lowering medication. If total serum cholesterol is all that is available, use >/= 200 mgdO (5.18mmolL)
Diabetes
Fasting Plasma Glucose >/= 126 mgdL (7.0mmoldL) or 2 h plasma glucose values in oral glucose tolerance test >/= 200mg*dL (11.1 mil * L) or HbA1C >/= 6.5%
HDL-C (Only Negative Risk Factor)
> /= 60 mgdL (1.55mmolL)
1st degree Relative
Parent, Sibling, or Offspring
Atherosclerosis Cardiovascular Disease Risk Factors and Defining Criteria Table
Should not be viewed as an all-inclusive list; contains clinically relevant established CVD risk factor criteria that should be considered collectively when making decisions about risk for development or progression of a cardiopulmonary or metabolic disease (No longer used as part of the screening process prior to exercise)
If the presence or absence of a CVD risk factor is NOT disclosed or it is not available…
That CVD risk factor should be counted for as a risk factor!!!
